"AI integration in healthcare systems: opportunities and governance challenges for India"
Ayushman Bharat Digital Mission (ABDM)
India's national digital health infrastructure linking health records, IDs, and services across providers.
Ambient scribing
AI-enabled real-time transcription of clinical consultations into structured medical records, reducing documentation burden on doctors.
India's healthcare system faces a structural mismatch: a rapidly growing and diversifying disease burden — spanning communicable diseases, non-communicable conditions such as diabetes and cardiac disease, and cancer — set against a healthcare workforce and infrastructure that cannot expand at the same pace. Specialist care remains heavily concentrated in metropolitan centres, leaving secondary and tertiary towns underserved. The National Health Policy and the Ayushman Bharat programme have sought to address access gaps through insurance coverage and health and wellness centres, but the underlying challenge of expertise distribution — getting the right clinical knowledge to the right patient at the right time — remains unsolved by physical infrastructure alone.
Artificial Intelligence, as a class of technology, endeavours to simulate cognitive functions — learning, pattern recognition, and decision-making — in machines, enabling them to perform tasks with minimal human intervention. In healthcare, this translates into tools that can read diagnostic images, flag deteriorating patients, assist documentation, and support clinical decision-making. The NCERT framework describes AI as building a knowledge base from data to replicate human reasoning; in medicine, the data is clinical — imaging, lab values, patient histories — and the reasoning is diagnostic or prognostic. The convergence of large clinical datasets, improved computing, and India's expanding digital health infrastructure has brought this possibility from research settings into operational healthcare.
AI in healthcare operates across several distinct functional layers. In diagnostics, machine-learning models trained on radiology images can prioritise scans, flag anomalies, and assist radiologists — particularly valuable where radiologist-to-population ratios are low. In clinical decision support, AI tools can alert care teams to early signs of patient deterioration, reducing adverse events in hospital settings. In administrative and operational functions, AI can automate appointment scheduling, clinical documentation (ambient scribing), claims processing, and inventory management — tasks that currently consume significant clinician time and contribute to burnout.
The Ayushman Bharat Digital Mission (ABDM) provides the foundational digital layer for AI deployment in India. By linking over 100 crore health records to Ayushman Bharat Health Accounts, ABDM has created a longitudinal data infrastructure that AI systems require to function at population scale. …
