"India's public health policies drift from population needs toward individual wellness"
Universal Health Coverage (UHC)
ensuring every individual accesses needed health services without financial hardship.
Health Promotion
population-based approach addressing social, economic, and environmental determinants of health outcomes.
India's public health system is built on a three-tier infrastructure: sub-centres (SCs), primary health centres (PHCs), and community health centres (CHCs). These institutions were designed under the district health system to deliver preventive, promotive, curative, and rehabilitative care to the population. The National Health Policy, 2017 committed India to achieving Universal Health Coverage (UHC), ensuring every individual can access needed health services without financial hardship. UHC is a globally recognised goal, endorsed by the World Health Organization (WHO) and embedded in the Sustainable Development Goals (SDG 3.8).
India's public health spending has historically been low, hovering around 1.3–1.5% of GDP, well below the 2.5% target set by the National Health Policy, 2017. This chronic underfunding has produced a dual burden: rising costs in the private sector and poor quality in the public sector. The Ayushman Bharat programme, launched in 2018, was intended to address both supply-side gaps (through Health and Wellness Centres) and demand-side barriers (through the Pradhan Mantri Jan Arogya Yojana, a publicly funded health insurance scheme). More recently, the Ayushman Bharat Digital Health Mission (ABDHM) has added a digital layer to this architecture.
The Ayushman Bharat Health and Wellness Centres (AB-HWCs) were introduced in 2018 by rebranding existing SCs, PHCs, and CHCs with a mandatory "Health and Wellness Centre" prefix. The intent was to expand the scope of primary care beyond maternal and child health to include non-communicable diseases, mental health, and palliative care. However, the renaming created institutional ambiguity. Each tier of the original three-tier system had a distinct mandate evolved over decades. Collapsing their identities under a single prefix blurred these mandates for both health professionals and policymakers.
A deeper conceptual problem underlies this shift. The "wellness" framing draws on a tradition that originated in the 1950s wellness movement, which conceptualised health beyond biological dimensions to include mental, spiritual, social, and environmental well-being. The WHO itself defined health as "not merely the absence of disease," promoting positive well-being. Over time, wellness expanded into an individualistic concept, placing primary responsibility on individuals to modify their health-related choices. …
